The disconnects in the system are myriad: physician to patient, health authority to physician, long term care to physician, long term care to hospital, physician to pharmacy, and family physician to specialist. Tracking down medication changes, what was actually prescribed/filled/taken and when, which tests were done, which tests are pending–consumes way too much of my day. Notification systems from the hospital to the patient for diagnostic services are terrible–patients come in saying “I have a test at the hospital on day X” and they don’t know which test, what time it is, who ordered it, what the requisite test prep is. My staff and I seem to moonlight as detectives, trying to figure out the story and the plan from info cobbled together from multiple systems, rather than having a solid understanding of the patient journey, whether initiated by our office or a specialist. Not only that, but sometimes we can’t even access the systems!!! After multiple emails and calls to careconnect, we STILL can’t access pharmanet! Getting real-time support when I have time to troubleshoot is next to impossible. I am getting too old for this and it is going to drive me to leave practice early. I’m close to 50, I could have another 10 years in me, but at this rate I’ll be lucky to survive 2.
