Duplicates referrals the patient was unaware of, duplicate lab testing, delayed results therefore additional unnecessary testing but due to lack of information at the time repeated. Depending on the case can be hours for me to call specialist office, request MOA to request transfer of information from specialist office or hospitals, lifelabs. That time would otherwise go to patient care.
