I access patient information from 3-4 different systems on a daily basis – MedAccess, Powerchart, MOiS, eHealth. I deliver FP-OB services in Prince Rupert Regional Hospital, which serves a catchment that includes four Coast Ts’msyen communities in Northwestern BC. Our maternity clinic uses MOiS. The hospital uses Cerner/Powerchart. The villages are under FNHA. They currently paperchart and RNs in the nursing stations do not have access to any patient information that isn’t faxed to the nursing station and entered into paper charts. Eventually, FNHA will have MOiS, but a different instance than in Prince Rupert. The fragmentation of information is particularly noticeable at transitions of care, for example when a patient is discharged from hospital after giving birth and goes home where RNs are to provided post partum follow up for the dyad. The incomplete transfer of information can hinder the delivery of appropriate medical care.
