A significant portion of my day—often at least an hour—is spent navigating multiple disconnected systems, including CareConnect, PharmaNet, direct phone calls to pharmacies, and requesting specialty consultation notes to obtain information that should be readily available. As a U.S.-trained family physician who recently returned home to serve my community in British Columbia, I came from a healthcare system where comprehensive patient records were accessible through a unified electronic medical record. It has been disheartening to transition to a system where essential clinical information is fragmented across multiple platforms. One recent example involved a patient presenting with palpitations after an emergency department visit. Despite reviewing all available resources, I was unable to read the emergency department discharge summary as it was illegible or determine what investigations had already been completed. As a result, I had to repeat an ECG and bloodwork because I could not confidently determine the patient’s previous workup. Unfortunately, this is not an isolated incident. Because no single platform provides a comprehensive longitudinal medical record, I often spend hours piecing together laboratory results, imaging reports, consultation notes, and medication records to understand a patient’s medical history. This fragmented system takes valuable time away from patient care, contributes to unnecessary duplicate testing, delays clinical decision-making, and increases healthcare costs.
