Digital systems are getting more disconnected and worse. The worst case scenario is when a patient has been to hospital and I am not recorded as their family doctor. Then when they come in I have to log in to CareConnect and start clicking on everything. Eventually I find a discharge summary that is copypasta. There are autopopulated fields everywhere, there is no thought process to the summary and I have no idea what medications the patient is on or why. Nowadays I have to consult a primary care pharmacist just to understand what happened to medications in hospital. It adds 15-30 minutes the day of the visit and another 15-30 minutes when I confer with the pharmacist. All to get back to what it was like before the hospital EHR transition.
